1. Your funnel
Spend includes ads, agency fees, directory listings and the cost of anything you pay to generate leads. Collected means money actually received after insurance write-offs and unpaid balances.
2. Your patient types
Cost per visit is what delivering one visit costs you: provider time, supplies and room share. Shares are relative, so 50, 35, 15 and 5, 3.5, 1.5 give the same answer. You can paste rows from a spreadsheet (tab separated).
Enter your funnel and at least one patient type, then click Calculate.

What this does not cover
- Referrals and word of mouth. Patients who arrive free lower your true cost; enter only paid leads to see the paid channel on its own.
- Reactivations and maintenance plans beyond the visits you enter per patient.
- Insurance denials and timing. Enter the amount you actually collect, not the billed amount.
- Clinical appropriateness. Margin is never a reason to recommend a number of visits. Care plans belong to the treating clinician and the patient.
- Taxes and owner pay. Everything is before income tax.
Common questions
What is a good cost to acquire a new chiropractic patient?
It depends on what a patient is worth. Multiply the visits a new patient completes by what you collect per visit, subtract the cost of delivering those visits, and that margin is the most you can spend. Many clinics aim to spend no more than a quarter to a third of it; the calculator shows the ceiling for the return you choose.
How do I calculate chiropractic patient lifetime value?
Take the visits in a typical episode of care, multiply by the amount actually collected per visit after insurance write-offs and unpaid balances, add the new-patient exam, then subtract the per-visit cost of delivering care. Do it per patient type, since cash care-plan, insurance and injury patients differ.
What is a good show rate for new chiropractic patients?
Show rate is the share of booked first visits that attend. Below about 70 percent, reminders and a confirmation call usually cost less than the marketing needed to replace the missed visits, because attended patients lower acquisition cost without extra spend.
Does this work for physical therapy, acupuncture or massage clinics?
Yes. Any practice that wins new patients through paid or organic leads, sees them for a course of visits and collects a per-visit amount can use the same segments: name, share of new patients, visits, collected per visit and cost per visit.
Is this financial advice?
No. It does arithmetic on the numbers you enter. It is not accounting, tax, legal or medical advice, and it does not tell you how to treat or what to charge a patient.
